ABSTRACT Purpose The objective of this study was to validate the Multiprofessional Screening Instrument for Broncho-aspiration Risk in Hospital Environment, which is aimed at the elderly population, based on response processes. Methods Judges applied the instrument to different patients and randomly selected. After the application, the judges were interviewed so that it was possible to verify their impression regarding the relevance of the items about their interpretation of the written content, as well as grammatical and semantic issues. Suggestions for adding alternative questions and answers were considered, as well as proposals for adapting the questions that made up the instrument. Non-verbal reactions, such as facial expressions that suggested doubts or hesitations, by the judges concerning the instrument were also analyzed. Results The agreement of the judges concerning each item of the device was calculated by the Content Validity Index (CVI) and by the Intraclass Correlation Coefficient (ICC), and their results showed a high level of agreement. Through the suggestions of the judges, a new version of the Multi-professional Screening Instrument for the Risk of Broncho-aspiration in a Hospital Environment in the Elderly was elaborated. Conclusion The results obtained showed that the validity of the Multi-professional Instrument for Screening the Risk of Broncho-aspiration in the Hospital Environment with the elderly population, based on the response processes, was achieved and makes it a promising device to assist professionals in hospital care for the elderly.

RESUMO Objetivo O objetivo deste estudo foi validar, com base nos processos de resposta, o Instrumento Multiprofissional de Rastreio para o Risco de Broncoaspiração em Ambiente Hospitalar, direcionado à população idosa. Método O instrumento foi aplicado por juízes em pacientes distintos e selecionados de forma aleatória. Após a aplicação, os juízes foram entrevistados para que fosse possível verificar a impressão deles quanto à relevância dos itens e quanto a interpretação sobre o conteúdo escrito, bem como a questões gramaticais e semânticas. Foram consideradas sugestões de acréscimo de alternativas de perguntas e de respostas, além de propostas de adequação de questões que compunham o instrumento. As reações não verbais, tais como expressões faciais que sugeriram dúvidas ou hesitações, por parte dos juízes, em relação ao instrumento, também foram analisadas. Resultados A concordância dos juízes em relação a cada item do dispositivo foi calculada pelo Índice de Validade de Conteúdo (IVC), e pelo Coeficiente de Correlação Intraclasse (CCI), sendo que seus resultados demonstraram alto nível de concordância. Através das sugestões dos juízes, elaborou-se uma nova versão do Instrumento Multiprofissional de Rastreio para o Risco de Broncoaspiração em Ambiente Hospitalar à população idosa. Conclusão Os resultados obtidos demonstraram que a validade do Instrumento Multiprofissional para Rastreio do Risco de Broncoaspiração em Ambiente Hospitalar junto à população idosa, baseada nos processos de respostas, foi alcançada.

ABSTRACT Purpose To understand the meanings that the therapeutic bond assumes for clinical speech therapists. Methods The research was approved by the Ethics Committee, being of a transversal character, with a quantitative-qualitative approach in the Content Analysis. The research with the participation of 96 clinical speech therapists, registered in the Speech Therapy Council of the 3rd region (CRFa 3), which covers the States of Paraná and Santa Catarina. Results Of the 96 speech therapists included, a significant part of the participants defined the therapeutic bond as a relationship/interaction. Regarding the role of the bond for the speech therapy clinical work, most professionals declared theirs as a fundamental basis and another part of the bond is necessary for the evolution/development of the patient. Conclusion It is possible to understand that, according to the therapeutic patients, it is essential to sustain, maintain the clinical work for users, impacting the resignification of the complaint and the minimization of the users' suffering.

RESUMO Objetivo Compreender os sentidos que o vínculo terapêutico assume para fonoaudiólogos clínicos. Método A pesquisa foi aprovada por Comitê de ética, sendo de caráter transversal, de abordagem quanti-qualitativa, pautada na Análise do Conteúdo. A pesquisa contou com a participação de 96 profissionais fonoaudiólogos clínicos, inscritos no Conselho de Fonoaudiologia da 3ª região (CRFa 3), o qual abrange os Estados do Paraná e de Santa Catarina. Resultados Dos 96 fonoaudiólogos incluídos, parte significativa dos participantes definiram o vínculo terapêutico como sendo relação/interação. Referente ao papel do vínculo para o trabalho clínico fonoaudiológico, a maioria dos profissionais o descreveu como base fundamental e outra parte deles afirmou que tal vínculo é necessário para a evolução/desenvolvimento do paciente. Conclusão É possível compreender que, de acordo com os participantes, a relação terapêutica é essencial para a sustentação e manutenção do trabalho clínico fonoaudiológico, impactando na ressignificação da queixa e na minimização do sofrimento dos usuários.

ABSTRACT Purpose: to map, through a literature survey, which instruments are used to assess swallowing in patients after orotracheal extubation. Methods: available evidence was mapped through six electronic databases and gray literature. There were no restrictions regarding gender, ethnicity of the individuals, language of the studies, time of publication, and diagnosis. Results: the most mentioned protocol in the studies was the Dysphagia Risk Evaluation Protocol and the most cited objective assessment exam was the flexible endoscopic evaluation of swallowing. Conclusion: there is a need for a specific protocol to evaluate this profile of patients, in addition to comparative studies of subjective clinical evaluation and instrumental imaging.

ABSTRACT Objective: This systematic review aims to answer the following focus question: “Is there an association between atypical swallowing and malocclusions?”. Methods: Appropriate word combinations were chosen and tailored specifically for each of the following electronic databases: EMBASE, Latin American and Caribbean Literature in Health Sciences (LILACS), LIVIVO, PubMed/Medline, Scopus, Web of Science, and gray literature, without any restrictions, up to February 2021. According to the selection criteria, only cross-sectional studies were included. The following inclusion criteria were considered: a sample composed of children, adolescents, and adults; patients clinically diagnosed with atypical swallowing; patients with normal swallowing; and outcome of interest of atypical swallowing in patients with malocclusion. The data consisted of study characteristics, sample characteristics, results, and conclusion of each study. The risk of bias was assessed using the JBI Critical Appraisal Checklist for Analytical Cross-Sectional Studies, and the certainty of evidence was assessed using the GRADE tool. Results: 4,750 articles were identified. After a two-step selection, four studies were included. A higher frequency of distal occlusion, extreme maxillary overhang, and open bite was related to swallowing disorders; most studies pointed to posterior crossbite as a malocclusion more associated with atypical swallowing. All studies had a moderate to high risk of bias, and the certainty of evidence was very low. Conclusion: The results indicate that atypical swallowing is associated with malocclusions and that posterior crossbite is the main malocclusion found, but only in the young population (3-11 years). RESUMO Objetivo: A presente revisão sistemática tem como objetivo responder à seguinte questão focal: “Existe associação entre deglutição atípica e más oclusões?”. Métodos: Combinações de palavras e truncamentos apropriados foram adaptados para as bases de dados eletrônicas: EMBASE, Literatura Latino-Americana e do Caribe em Ciências da Saúde (LILACS), LIVIVO, PubMed/Medline, Scopus e Web of Science e a literatura cinzenta, sem qualquer restrição, até fevereiro de 2021. Os critérios de inclusão foram: estudos transversais; amostra composta por crianças, adolescentes e adultos; pacientes com diagnóstico clínico de deglutição atípica; pacientes com deglutição normal, e o desfecho de interesse foi deglutição atípica em pacientes com má oclusão. Os dados extraídos de cada estudo foram características do estudo, da amostra e a conclusão. O risco de viés foi avaliado usando a Lista de verificação de avaliação crítica da JBI para estudos transversais analíticos, e a certeza das evidências foi avaliada usando a ferramenta GRADE. Resultados: Foram identificados 4.750 artigos. Após uma seleção em duas fases, quatro estudos foram incluídos. Maior frequência de disto-oclusão, protuberância maxilar extrema e mordida aberta foram relacionadas a distúrbios da deglutição, com a maioria dos estudos apontando para mordida cruzada posterior como a má oclusão mais associada à deglutição atípica. Todos os estudos tiveram um risco de viés moderado a alto, e a certeza das evidências foi considerada muito baixa. Conclusão: Os resultados indicam que a deglutição atípica está associada à mordida cruzada posterior apenas na população jovem de 3 a 11 anos.

Abstract Introduction Parkinson disease (PD) is a progressive degeneration characterized by motor disorders, such as tremor, bradykinesia, stiffness and postural instability. Objective To evaluate the independence, confidence and balance in the development of daily activities in patients with PD before and after rehabilitation. Methods A descriptive, retrospective cross-sectional study was carried out with 16 patients (mean 57.6 ± 18.7 years), submitted to anamnesis, otolaryngological evaluation and vestibular assessment. The Vestibular Disorders Activities of Daily Living (VADL) and the Activities-Specific Balance Confidence (ABC) scales were applied before and after rehabilitation with virtual reality. Results a) The instrumental subscale of the questionnaire showed statistically significant result (p = 0.022; 95 % CI 1.21; 2.21) between the first and second assessments; b) The correlation between the questionnaires showed statistically significant result in the ambulation subscale (p = 0.011; 95 % CI -0.85; -0.17) first and (p = 0.002, 95 % CI -0.88; -0.31) second assessments, and the functional subscale was only verified in the second assessment (p = 0.011, 95 % CI -0.85; -0.17); and c) The patients presented clinical improvement in the final assessment after rehabilitation with significant result for the tightrope walk (p = 0.034, 95 % CI -12.5; -0.3) and ski slalom games (p = 0.005, 95 % CI -34.8; -6.6). Conclusions Our results showed that the VADL and ABC questionnaires, applied before and after rehabilitation, were important tools to measure the independence, confidence and balance while developing daily activities. The VADL and ABC questionnaires may effectively contribute to quantify the effect of the applied therapeutics and, consequently, its impact on the quality of life of patients with PD.

RESUMO Objetivo: demonstrar a viabilidade do uso da análise acústica dos sons da deglutição como método conjugado à avaliação clínica em pacientes traqueostomizados acometidos por traumatismo cranioencefálico. Métodos: estudo observacional do tipo transversal. Participaram deste estudo 10 pacientes adultos, com idade média de 43,6 anos internados em leito hospitalar, unidades de terapias intensivas ou semi-intensivas, no período de maio a julho de 2016. O critério para inclusão foi ser acometido por traumatismo cranioencefálico, confirmado por tomografia axial computadorizada. Foi aplicado o Protocolo Fonoaudiológico de Decanulação Traqueal para a avaliação clínica e a ausculta cervical por meio do Sonar Doppler. Foi aplicado o Test Fischer e não foi encontrada relação significante (p>0,05), entre as variáveis analisadas nas duas consistências e na decanulação. Resultados: foi observada a presença de sinal acústico de elevação laríngea, presença de ruído entre as deglutições, presença de sinal acústico sugestivo de resíduo em 50% dos pacientes, para as consistências testadas. Quando correlacionados a relação entre frequência de pico, tempo médio da onda, presença de resíduo entre as deglutições, sinal acústico sugestivo de resíduos e decanulação (Tabela 5), verificou-se que não existe correlação significante (p>0,05) entre as variáveis analisadas nas duas consistências e a decanulação. Conclusão: o estudo sugere que o uso do Sonar Doppler é viável como método conjugado na avaliação clínica das disfagias para a decanulação de pacientes acometidos por traumatismo cranioencefálico.

ABSTRACT Purpose: to demonstrate the feasibility of using the acoustic analysis of swallowing sounds as a combined method in the clinical assessment of tracheostomized patients affected by traumatic brain injury. Methods: an observational, cross-sectional study. A total of 10 adult patients, in the mean age of 43.6 years, participated. They were hospitalized in wards, semi-intensive, or intensive care units, from May to July 2016. The inclusion criterion involved being affected by traumatic brain injury, confirmed with a CT scan. The Speech Therapy Tracheal Decannulation Protocol was used in the clinical assessment, as well as the cervical auscultation with the Doppler sonar. Fisher’s test was used, resulting in no significant relationship (p > 0.05) between the variables analyzed in the two consistencies and decannulation. Results: the presence of an acoustic signal of laryngeal elevation was observed, as well as noise between the swallowings, and acoustic signal suggestive of residue in 50% of the patients for the consistencies tested. When the peak frequency, mean wave time, presence of residue in between swallowings, and acoustic signal suggestive of residues were correlated with decannulation (Table 5), no significant correlation was verified (p > 0.05) between the variables analyzed in the two consistencies and the decannulation. Conclusion: the study suggests that it is feasible to use the Doppler sonar as a combined method in the clinical assessment of dysphagia for the decannulation of patients affected by traumatic brain injury.

Resumo Introdução: A disfagia orofaríngea é uma comorbidade altamente prevalente em pacientes neurológicos e representa uma séria ameaça à saúde, pode levar a desfechos como pneumonia por aspiração, hospitalização e até morte. A avaliação propõe um método não invasivo, acústico, para diferenciar entre indivíduos com e sem sinais de penetração e aspiração. Objetivo: Esta revisão sistemática analisou a validade diagnóstica de diferentes métodos para avaliação dos sons de deglutição, quando comparados com a videofluoroscopia da deglutição para detectar disfagia orofaríngea. Método: Artigos nos quais o objetivo principal era avaliar a acurácia dos sons de deglutição foram pesquisados em cinco bancos de dados eletrônicos sem limitações de idioma ou tempo de publicação. As medidas de acurácia descritas nos estudos foram transformadas para construir curvas ROC (Receptor Operating Characteristic) e gráfico em floresta (forest plot) com o auxílio do software Review Manager v. 5.2 (The Nordic Cochrane Centre, Copenhagen, Dinamarca). A metodologia dos estudos selecionados foi avaliada com a ferramenta Avaliação da Qualidade de Estudos de Acurácia de Testes Diagnósticos-2. Resultados: A busca eletrônica final resultou na identificação de 554 artigos; no entanto, apenas três estudos preencheram os critérios de inclusão. Os valores de acurácia (área abaixo da curva) foram 0,94 para microfone, 0,80 para doppler e 0,60 para estetoscópio. Conclusão: Baseado nas evidências limitadas e da baixa qualidade metodológica, pois foram poucos os estudos incluídos, e com pequeno tamanho amostral, de todos os testes diagnósticos (index testes) encontrados para essa revisão sistemática o doppler mostrou excelente acurácia diagnóstica na discriminação dos sons de deglutição, o microfone demonstrou uma boa acurácia na discriminação dos sons de pacientes disfágicos e o estetoscópio revelou o melhor teste de triagem.

Abstract Introduction: Oropharyngeal dysphagia is a highly prevalent comorbidity in neurological patients and presents a serious health threat, which may lead to outcomes of aspiration pneumonia, ranging from hospitalization to death. This assessment proposes a non-invasive, acoustic-based method to differentiate between individuals with and without signals of penetration and aspiration. Objective: This systematic review evaluated the diagnostic validity of different methods for assessment of swallowing sounds, when compared to videofluroscopy swallowing study to detect oropharyngeal dysphagia. Methods: Articles in which the primary objective was to evaluate the accuracy of swallowing sounds were searched in five electronic databases with no language or time limitations. Accuracy measurements described in the studies were transformed to construct receiver operating characteristic curves and forest plots with the aid of Review Manager v. 5.2 (The Nordic Cochrane Centre, Copenhagen, Denmark). The methodology of the selected studies was evaluated using the Quality Assessment Tool for Diagnostic Accuracy Studies-2. Results: The final electronic search revealed 554 records, however only 3 studies met the inclusion criteria. The accuracy values (area under the curve) were 0.94 for microphone, 0.80 for doppler, and 0.60 for stethoscope. Conclusion: Based on limited evidence and low methodological quality because few studies were included, with a small sample size, from all index testes found for this systematic review, doppler showed excellent diagnostic accuracy for the discrimination of swallowing sounds, whereas microphone-reported good accuracy discrimination of swallowing sounds of dysphagic patients and stethoscope showed best screening test.

RESUMO Objetivo: validar em aparência e conteúdo, o “manual de orientação para pacientes adultos com disfagia em terapia fonoaudiológica em home care”. Métodos: estudo qualitativo e quantitativo, dividido em três etapas; a primeira, com a construção do material piloto; na segunda etapa foram escolhidos 8 juízes especialistas na área, com mestrado ou doutorado, os quais avaliaram o manual por meio de um questionário; e a terceira etapa foi direcionada à escolha de 8 juízes cuidadores de pacientes em atendimento home care, como parte representante da população em estudo. Resultados: na análise das respostas dos juízes, utilizou-se a correlação linear de Pearson, e considerando válidas as respostas com correlação maior que 0,6, sendo este um parâmetro que indica forte correlação linear. Conclusão: os resultados encontrados, após a avaliação dos juízes evidenciaram a existência e correlação maior que 0,6 em todas as questões avaliadas, demostrando existir forte correlação entre as perguntas referentes ao conteúdo do manual, e sugerem validade de aparência e conteúdo.

ABSTRACT Purpose: to validate as to appearance and content, the "guidance manual for adult patients with dysphagia in home care speech therapy". Methods: a qualitative and quantitative study, divided into three stages; first, the pilot material was constructed; second, 8 expert judges, with a master or doctorate degree, were chosen to assess the manual through a questionnaire; and third, 8 caregiver judges of home care patients were chosen to represent the study population. Results: Pearson's linear correlation was used in the analysis of the judges' answers, considering the responses with a correlation greater than 0,6 which is a parameter that indicates a strong linear correlation. Conclusion: the results found, after the judges' evaluation, showed a strong correlation between the questions regarding the content of the manual and suggesting appearance and content validity.

RESUMO Objetivo Descrever o efeito da conduta fonoaudiológica no processo de decanulação traqueal em indivíduos com traumatismo cranioencefálico (TCE). Método Estudo clínico transversal prospectivo controlado. Participaram deste estudo dois grupos de indivíduos com TCE confirmado por tomografia axial computadorizada, sendo o grupo de estudo (G1) e o grupo controle (G2) compostos por 30 indivíduos cada, com 25 (83,3%) indivíduos do gênero masculino e 5 (16,7%) do gênero feminino em cada grupo. A faixa etária variou de 18 a 53 anos, com média de 32 anos. Foi elaborado um instrumento de avaliação fonoaudiológica para indivíduos com TCE traqueostomizados composto por investigação do nível de consciência, cognição e deglutição (anexo 1) e conduta. O G1 recebeu a avaliação proposta pelo estudo e o G2, análise retrospectiva de prontuário sem avaliação fonoaudiológica. As variáveis, tempo de permanência com a traqueostomia e total de dias de internamento foram os marcadores de mensuração do efeito da conduta fonoaudiológica com esse instrumento nessa população. Resultados Verificou-se que o G1 obteve uma redução média de 4,2 dias de permanência com a traqueostomia e de 4,4 dias no tempo de internamento hospitalar quando comparado com G2, porém sem significância estatística (p = 0,2031). Conclusão O grupo que foi avaliado e recebeu a conduta fonoaudiológica proposta no instrumento obteve diminuição do tempo de permanência com a traqueostomia bem como redução do tempo de internamento hospitalar.

ABSTRACT Purpose To describe the effect of Speech-Language Pathology (SLP) management on the tracheal decannulation process in patients with traumatic brain injury (TBI). Methods Prospective controlled clinical study. Two groups of patients with TBI confirmed by computed axial tomography were included in the study group (G1) and control group (G2) composed of 30 individuals each, with 25 (83.3%) male and 5 (16.7%) female individuals in both groups. Patients’ age ranged from 18 to 53 years old – mean age was 32 years. A SPL assessment tool was developed for tracheostomized patients with TBI, composed of investigation of awareness level, cognition and swallowing (annex 1) and conduct. G1 underwent the assessment proposed by the study, and G2 was assessed by retrospective analysis of medical records without SLP evaluation. In this population, the variables time with tracheostomy and total days of hospitalization were the measurement markers for the effect of SLP conduct with this instrument. Results It was verified that G1 presented mean reduction of 4.2 days with tracheostomy and of 4.4 days in length of hospital stay when compared to G2. However, these figures are not statistically significant (p = 0.2031). Conclusion The group that was evaluated and received the SLP conduct proposed in the instrument presented a reduction in the time of permanence with tracheostomy, as well as in hospital stay.

RESUMO Trata-se de revisão integrativa com objetivo de conhecer a produção científica e analisar as estratégias acerca da comunicação do paciente traqueostomizado. A busca no portal da Biblioteca Virtual em Saúde (BVS), com os descritores "communication and tracheostomy", resultou em publicações das bases de dados Literatura Latino- Americana e do Caribe de Informação em Ciências da Saúde (LILACS), Literatura Internacional (MEDLINE) e Coleciona SUS. Foram considerados os artigos científicos publicados no período de 2001 a 2015, por representar o período com maior produção científica, que envolvessem adultos e, sem restrição quanto aos idiomas espanhol, inglês e português nem para o tipo do estudo. Foram selecionados 16 publicações, composta por 15 artigos e uma tese, as quais foram classificadas por categorias: conhecimento, comunicação, e recursos auxiliares. Diante das publicações analisadas, verifica-se que a preocupação acerca da comunicação do paciente traqueostomizado é tema crescente no contexto da produção científica nessa última década.

ABSTRACT It is an integrative review aiming at knowing the scientific production and analyze the strategies on the communication of the tracheostomized patient. The search on the site of the Virtual Health Library (Biblioteca Virtual em Saúde - BVS), with the descriptors "communication and tracheostomy", resulted in publications of the Latin American and Caribbean Health Sciences Literature (Literatura Latino-Americana e do Caribe de Informação em Ciências da Saúde - LILACS), International Literature (MEDLINE), and Coleciona SUS databases. Scientific articles published between 2001 and 2015 were considered, as they represented the period of the greatest scientific production involving adults, and unrestrictedly for Spanish, English and Portuguese idioms or the type of study. Sixteen (16) publications, comprising 15 articles and 1 thesis, were selected, which were classified by categories: knowledge, communication and supporting resources. From the analyzed publications, it is verified that the concern on the communication of tracheostomized patients has been an increasing theme in the context of the scientific production in the past decade.

RESUMO Objetivo O objetivo deste estudo foi analisar os efeitos imediatos do exercício de trato vocal semiocluído (ETVSO) com tubo LaxVox® em cantores. Método Participaram 23 cantores, estudantes de canto lírico, com idades entre 18 e 47 anos (média de 27,2 anos). Inicialmente, a coleta de dados envolveu a aplicação de um questionário contendo os dados de identificação e a gravação da vogal “ε”, contagem de números de um a dez e trecho de música do repertório atual. Na sequência, os participantes receberam orientações e realizaram o ETVSO com tubo LaxVox® por três minutos. Finalmente, as mesmas amostras vocais foram coletadas imediatamente após a execução e os cantores responderam a um questionário sobre sua autopercepção em relação às possíveis modificações vocais geradas pelo exercício. As amostras vocais foram analisadas por juízes fonoaudiólogos e professores de canto e por meio de análise acústica. Resultados A maior parte dos cantores referiu melhor emissão pós-exercício, tanto na fala quanto no canto. Na opinião dos juízes, não houve diferenças entre emissões pré e pós-exercício nas provas da avaliação perceptivo-auditiva (emissão sustentada, fala e canto). A análise acústica da vogal indicou aumento da Frequência Fundamental (F0) e redução da Proporção Glottal to Noise Excitation (GNE) pós-exercício. Conclusão O exercício de trato vocal semiocluído com tubo LaxVox® promove efeitos imediatos positivos quanto à autoavaliação e análise acústica da voz do cantor profissional sem queixas. Quanto à avaliação perceptivo-auditiva da fala e do canto, parece não haver modificações imediatas significativas.

ABSTRACT Purpose The purpose of this study was to analyze the immediate effects of the semi-occluded vocal tract exercise (SOVTE) using the LaxVox® tube in singers. Methods Participants were 23 singers, classical singing students, aged 18 to 47 years (mean age = 27.2 years). First, data was collected through the application of a demographic questionnaire and the recording of sustained emission - vowel /ε/, counting 1-10, and a music section from the participants’ current repertoire. After that, the participants were instructed and performed the SOVTE using the LaxVox® tube for three minutes. Finally, the same vocal samples were collected immediately after SOVTE performance and the singers responded to a questionnaire on their perception regarding vocal changes after the exercise. The vocal samples were analyzed by referees (speech-language pathologists and singing teachers) and by means of acoustic analysis. Results Most of the singers reported improved voice post-exercise in both tasks - speech and singing. Regarding the perceptual assessment (sustained vowel, speech, and singing), the referees found no difference between pre- and post-exercise emissions. The acoustic analysis of the sustained vowel showed increased Fundamental Frequency (F0) and reduction of the Glottal to Noise Excitation (GNE) ratio post-exercise. Conclusion The semi-occluded vocal tract exercise with LaxVox® tube promotes immediate positive effects on the self-assessment and acoustic analysis of voice in professional singers without vocal complains. No immediate significant changes were observed with respect to auditory-perceptual evaluation of speech and singing.

Abstract Introduction Dysphagia is a common symptom in children with cerebral palsy, either in oral or pharyngeal phases. Children who face such difficulties tend to show health problems such as food aspiration, malnutrition and respiratory infections. Videofluoroscopic swallowing study is the most recommended for these cases, as it reveals the real situation during swallowing. Objective The study aimed to verify the occurrence of aspiration pneumonia in children with cerebral palsy after videofluoroscopy. Methods The population for this prospective cross-sectional study involved 103 children with cerebral palsy, referred for videofluoroscopic who had returned for medical examination after a week to search for signs and symptoms of pneumonia. Results The study involved 46 girls (44.66%) and 57 boys (55.34%), aged between 0 and 14 years of age. Of the total, 84 (81.5%) had dysphagia, of which 24 (23.3%) were severe, 8 (7.7%) were moderate and 52 (50.4%) were mild dysphagia. None of the children presented aspiration pneumonia or infectious complications during the course of videofluoroscopy or after the procedure. Conclusion In the population studied, the authors found no cases of aspiration pneumonia, even with tracheal aspiration present in 32 (31.07%) cases.

RESUMO Objetivos: avaliar e descrever os graus de capacidade e comprometimento funcionais em pacientes traqueotomizados de um hospital público e suas inter-relações. Métodos: estudo caso-controle, quantitativo, descritivo e transversal, realizado em um hospital terciário, vinculado ao Sistema Único de Saúde. A capacidade e comprometimento funcional foram avaliados por meio do Índice de Barthel e da Escala de Desempenho de Karnofsky, em pacientes traqueotomizados e não traqueotomizados em quatro unidades de internação. O grupo de estudo foi pareado com o grupo controle levando-se em conta a idade, o sexo e a doença que ocasionou a internação. Resultados: a amostra foi composta por 52 pacientes, 30 do sexo masculino e 22 do sexo feminino. A média de idade entre o grupo de estudo foi de 55,4 anos e no grupo controle de 55,1 anos. A análise estatística do Índice de Barthel no grupo de estudo classificou 84,62% dos pacientes como totalmente dependentes para a realização dos dez domínios do instrumento, ao contrário do grupo controle no qual somente 15,38% dos pacientes apresentaram este grau de dependência física e 53,85% mostravam total independência às mesmas atividades. O levantamento da escala Karnofsky evidenciou que 57,69% do grupo estudo apresentava escore de 40%, diferentemente do grupo controle no qual 42,31% obteve escore de 90%. Conclusão: houve implicação da capacidade e comprometimento funcional nos pacientes traqueotomizados, fato que demanda novo olhar da equipe multidisciplinar frente às incapacidades e demonstra que independente da traqueotomia facilitar o desmame da ventilação mecânica e consequentemente o período de internação, causa importante impacto na qualidade de vida.

ABSTRACT Purposes: to evaluate and describe the degrees of capacity and functional disability in tracheostomized patients of a public hospital and their interrelations. Methods: quantitative, descriptive, cross-cutting, case-control study carried out at a tertiary hospital from the Unified Health System. Capacity and functional disability were assessed using the Barthel Index and the Karnofsky Performance Scale in tracheostomized and non-tracheostomized patients in four hospital wards. The study group was matched with a control group taking into account age, gender and illness that led to hospitalization. Results: the sample consisted of 52 patients, 30 males and 22 females. The mean age of the study group was 55.4 years and in the control group, 55.1 years. Statistical analysis of the Barthel Index in the study group scored 84.62% of patients as totally dependent to perform the ten domains of the instrument, differing from the control group with only 15.38% of patients presenting this degree of physical dependence while 53.85% showed complete independence in the same activities. Karnofsky scale assessment showed that 57.69% of the study group had a 40% score, unlike the control group with 42.31% scoring 90%. Conclusion: there was capacity hindrance and functional disability in the tracheostomized patients, which demands a new look on the part of the multidisciplinary team confronting disabilities, and showing that despite tracheostomy facilitates weaning off mechanical ventilation and consequently the hospitalization period, it causes important impact on quality of life.

RESUMO: Objetivo: avaliar a qualidade de vida de pacientes com Acidente Vascular Encefálico e disfagia neurogênica. Métodos: estudo quantitativo, do tipo transversal, descritivo, realizado no Setor de Neurologia de um Hospital de Ensino, em Curitiba- Paraná. A amostra foi constituída de 35 indivíduos com Acidente Vascular Encefálico e queixa de disfagia. Os dados foram coletados por meio de questionário para avaliação da qualidade de vida em disfagia. Resultados: nas variáveis sociodemográficas houve predomínio de homens, idosos, brancos, casados e primeiro grau incompleto. A avaliação de qualidade de vida demonstrou que os domínios que apresentaram alterações foram os que investigam como a alteração da deglutição tem afetado o aspecto social dos participantes. A correlação de Mann-Whitney evidenciou significância estatística (p < 5) quando relacionou a deglutição como um fardo com o tempo de se alimentar (p 0,002), frequência dos sintomas (p <0,001), saúde mental (p <0,001) e fadiga (p <0,001). Conclusão: o levantamento estatístico comprovou o impacto causado pela disfagia neurogênica na qualidade de vida dos pacientes acometidos por Acidente Vascular Encefálico, representado pelas alterações encontradas nos resultados de avalição da qualidade de vida. Na correlação de Mann-Whitney, ao se realizar os cruzamentos entre os domínios do instrumento, os dados evidenciaram significância estatística quanto ao tempo de alimentação, medo de se alimentar, saúde mental, social e fadiga, que causam prejuízo na qualidade de vida dos pacientes com disfagia neurogênica.

ABSTRACT: Purpose: to assess quality of life in patients with cerebrovascular accident and neurogenic dysphagia. Methods: a quantitative, descriptive, cross-sectional study carried out by the Division of Neurology of a Teaching Hospital in Curitiba- Paraná State, Brazil. The sample consisted of 35 individuals with cerebrovascular accident and dysphagia. Data were collected through a questionnaire to assess quality of life in dysphagia. Results: as for the socio-demographic variables, predominantly males, elderly, white, married and incomplete primary education. The evaluation of quality of life showed that the domains that presented changes were the ones investigating how swallowing changes have affected participants' social aspect. Mann-Whitney correlation evidenced statistical significance (p <5) when related swallowing as a burden to feeding time (p 0.002), frequency of symptoms (p <0.001), mental health (p<0.001) and fatigue (p <0.001). Conclusion: the statistical survey confirmed the impact of neurogenic dysphagia on quality of life of patients suffering from cerebrovascular accident, represented by the changes found in the instrument used for rating the quality of life. In Mann-Whitney correlation, when performing intersections between the domains of the instrument, data showed statistical significance on feeding time, fear of eating, mental, social health and fatigue, which adversely affect the quality of life of patients with neurogenic dysphagia.

Resumo: OBJETIVO: avaliar a efetividade da realização de ações educativas em disfagia orofaríngea para a equipe de enfermagem pediátrica em um hospital de ensino. MÉTODOS: pesquisa avaliativa. O estudo foi realizado em hospital de referência terciária, vinculado ao Sistema Único de Saúde. A amostra foi constituída por 62 profissionais entre enfermeiros, técnicos e auxiliares de enfermagem que atuam na assistência à pacientes pediátricos. Aplicou-se um questionário para verificação do conhecimento prévio sobre disfagia, posteriormente foi realizada uma ação educativa e sua avaliação imediata. RESULTADOS: o conhecimento de enfermagem em disfagia evidenciou-se como positivo, principalmente no quesito do conceito com 96,77% de acertos, sintomas com 83,87% de acertos, causas com 74,19% de acertos, consequências com 70,97% de acertos, identificação do profissional habilitado para reabilitação com 85,48% de acertos e importância da reabilitação com 87,10% de acertos. Concernente ainda às respostas esperadas, ressaltam-se por não estarem de acordo: as fases da deglutição (50%), exames específicos (45,16%) e posição para alimentação via oral (32,26%). Justifica-se a necessidade da educação continuada, pois 85,48% dos participantes assinalaram que é importante a realização de treinamento. CONCLUSÃO: o estudo concluiu que o conhecimento fragmentado da enfermagem sobre disfagia, contudo, interessada e disposta para aprender, se for disponibilizado o conteúdo por meio da educação continuada. Haja vista, que os participantes da pesquisa após a ação educativa, expressaram a ampliação dos conhecimentos sobre disfagia, e reconheceram a importância destes, bem como expressaram o desejo de receber mais informações sobre essa e outras patologias.

Abstract: PURPOSE: to assess the effectiveness of carrying out educational activities on oropharyngeal dysphagia for pediatric nursing team at a teaching hospital. METHODS: it is an evaluation research. The study was carried out in a tertiary hospital from the National Health System. The sample consisted of 62 professionals including nurses, technicians and nursing assistants who deliver care to pediatric patients. A questionnaire was applied to check prior knowledge of dysphagia, and later was held an educational action and its immediate evaluation. RESULTS: nursing knowledge on dysphagia was evidenced as positive, especially the question on the concept with 96.77% accuracy, symptoms with 83.87% accuracy, causes with 74.19% accuracy, consequences with 70.97% correctness, identification of the professional qualified for rehabilitation with 85.48% accuracy, and importance of rehabilitation with 87.10% accuracy. Concerning the expected answers for yes, we emphasize the ones not in agreement: the phases of swallowing (50%), specific tests (45.16%) and position for oral nutrition (32.26%). The need for continuing education was justified as 85.48% of the participants reassured training importance. CONCLUSION: the study found that nursing knowledge on dysphagia, although fragmented, has had professionals interested and willing to learn if content is available through continuing education. Thus, research participants, after the educational activity, expressed the expansion of knowledge on dysphagia, and recognized its importance, also expressing a desire to receive more information about this and other pathologies.
